To submit payee information please follow the steps below. This is required for all individuals and entities that will be receiving payment or reimbursement from the University.
Steps to Submit the Payee Information Form
- Contact us at firstname.lastname@example.org to initiate the Payee Information Form. The form will be sent and completed through DocuSign.
- Prepare necessary supporting documents (i.e. photo ID or social security card; driver's license or passport is preferred) and upload through the DocuSign form.
